TMS Basics

What is TMS therapy?

TMS, or transcranial magnetic stimulation, is an FDA-cleared, non-invasive treatment for major depressive disorder. It uses gentle magnetic pulses to stimulate areas of the brain involved in mood regulation. It is medication-free and performed in our clinic.

How does TMS work?

A coil placed against your scalp delivers brief magnetic pulses, similar in strength to an MRI, to mood-regulating regions of the brain. Over a series of sessions, this repeated, focused stimulation is intended to help restore healthier patterns of brain activity. Because the pulses target a specific area, the rest of your body is not affected the way it can be with medication.

Is TMS FDA approved?

Yes. TMS is FDA-cleared for the treatment of major depressive disorder and certain other approved indications. The first rTMS device was cleared in 2008 for depression in people who had not responded to at least one antidepressant.

Is TMS the same as ECT?

No. Unlike ECT, TMS does not require anesthesia and is not associated with memory loss. You stay awake during sessions and can drive yourself home.

Eligibility

Am I a candidate for TMS?

TMS may be appropriate if you have major depressive disorder and have not found enough relief from therapy or medication. Only your provider and the TMS team can confirm eligibility after a screening and evaluation.

Do I need to have tried medication first?

Generally, TMS is considered when you have tried two or more antidepressants without the response you hoped for, though your provider makes the final determination based on your full history.

Can I have TMS if I have struggled with substance use?

You may still be considered when depression is your primary diagnosis and you are clinically stable. Your DTIP provider determines appropriateness individually.

Sessions

How long does a course of TMS take?

A typical course involves roughly 30 to 40 sessions over about 4 to 6 weeks, usually five days a week. The exact number varies based on your provider's recommendation, your progress, and insurance authorization.

How long is each TMS session?

Sessions are typically short, often around 20 to 30 minutes, depending on the protocol your provider uses. Some newer protocols are briefer. You remain awake and seated the entire time.

Does TMS hurt?

Most people tolerate TMS well. You may feel a tapping sensation or mild scalp discomfort during sessions, which often eases over time.

Can I drive after a session?

Yes. TMS does not require sedation, so you can drive yourself to and from appointments and return to your normal activities right away.

Safety

Is TMS safe?

TMS is generally safe and well tolerated. Your provider screens your medical history first to confirm it is appropriate for you.

What are the side effects?

The most common side effects are mild and temporary, such as scalp discomfort, headache, and fatigue. Serious side effects are uncommon, and your provider reviews the full picture with you.

Who should not have TMS?

Certain factors require careful screening, such as implanted devices near the head (pacemaker, cochlear implant, and similar), a history of seizures, or pregnancy. Your provider reviews these with you before treatment.

Can I keep taking my medication during TMS?

Often yes. Many people continue their current medications during TMS, but your provider reviews your full medication list and makes individualized recommendations as part of your treatment plan.

Insurance and Getting Started

Does insurance cover TMS?

How much does TMS cost?

Your out-of-pocket cost depends on your plan, including your deductible, copay or coinsurance, and whether we are in-network. We verify your benefits and review the expected cost with you before you commit, so it is not a surprise.

Do I need a referral to start TMS?

No. You can reach out directly for yourself or a loved one, and clinicians can also refer a patient. A screening and provider evaluation determine whether TMS is appropriate.
